​Thank you for your interest in Play for Patrick Heart Screens. Below you can register to volunteer or be screened at an upcoming event. If you do not see any events listed, they are not yet open for registration. Check back or sign up for our newsletter at https://playforpatrick.org .

​Volunteer Registration

​Please follow the following steps to volunteer:

1. Click the "Volunteer" button on this page and complete the required questions.
2. Click on the "Events" tab at the top of the screen and select an event.
3. Click the "Volunteer Registration" button.
4. Click the "Sign up" button next to the volunteer slot you would like to volunteer for.
5. You will receive an email confirmation of your registration.

​Participant Registration

To register a 14-24 year old to be screened:

1. Click on the "Participate" button below and select an event.
2. Click the "Participant Registration" button.
3. Select the appropriate gender to view available time slots OR if you have already filled out a registration form, click the "I have already filled out a registration form" and choose the participant.
4. Select a time slot and click "Sign up".
5. You will receive an email confirmation of your registration.
6. Print out the Heart Health Survey from the "My Heart Health History" tab and bring to the screen!

Need to change your registration?

​​​​​​To modify your registration, navigate to My Volunteer Registrations or My Participant Registrations. Click Cancel on your registration and use the Events page to register for a new event slot.
